Lean, often known as “purple drank” or “sizzurp,” is a recreational beverage that typically combines prescription-strength cough syrup containing codeine and promethazine with a soft drink and sometimes candy. What began as a cultural trend in certain music scenes has now become a widespread concern for public health professionals.

What Is Lean?

Lean is a concoction made by mixing codeine-promethazine cough syrup with soda, often a fruity flavor. Some users also add hard candy for sweetness. The codeine in the drink is an opioid, and promethazine is an antihistamine that enhances the sedative effects. Together, they produce a sense of euphoria, relaxation, and drowsiness.

Why Is Lean Dangerous?

The effects of Lean might seem mild at first, but the risks are serious. Codeine is an opioid, and like other opioids, it can be addictive and dangerous when misused. Promethazine, while not addictive on its own, can amplify the depressant effects of codeine.

Prolonged use of Lean can lead to:

  • Dependency and addiction

  • Seizures
  • Liver damage (especially if combined with acetaminophen-containing products)
